The Children’s Community Nursing Team

The Children’s Community Nursing Team are a team of very experienced nurses. They provide support, advice and direct nursing care to children aged 0-18 years.

The main aims of the service includes:

• Supporting children, young people and their families with both acute and chronic medical and surgical conditions.
• Support premature babies requiring home oxygen and enteral feeding.
• Assist in establishing care packages in partnership with education and social care.
• Provide training and teaching to patients, carers, schools, nurseries or any community setting.
• Act as the child’s and families advocate; attend meetings with the family to offer support and liaise with the other service providers.

They can also provide a limited service for end of life care.
