1. The NHS and how we fit

The National Health Service (NHS) is a very large part of the public sector. A full list of all the NHS Trusts can be found on http://www.nhs.uk/.

2. Who we are

The Trust offers a wide range of clinical services to the local population of Nottingham, and patients from further away. We work in close collaboration with primary care trusts and local general practitioners (GPs).  

Our website - www.nuh.nhs.uk - shows how we fit into the NHS across the area we serve. The Trust’s Annual Plan describes how we intend to meet national objectives and local health needs and the Annual Report gives a yearly update about our achievements

Organisation of the Trust

The Trust Board of Directors oversees the work of this organisation. Our Trust Board has six Non-Executive Directors, including the Chairman, and five Executive Directors. These Directors have voting rights and legal responsibility for oversight of the work of the Trust. The full membership of the Board is shown below.

Working in partnership with other organisations

We work in partnership with local communities, alongside our colleagues in the NHS, local authorities and other organisations in National Service Framework (NSFs) Local Implementation Schemes in Older People, Diabetes, and Coronary Heat Disease. Further information about NSFs can be found on the Department of Health’s website

The Trust has a major role to play in education and research and will continue the particularly strong links with the University of Nottingham established by its predecessor Trusts. The wide-ranging expertise in many aspects of clinical care has developed through teaching and research. Some of these partnerships will involve sharing information, with agreed protocols and procedures between organisations regulating how this information is shared.

The Trust Board receives a monthly update on the financial position of the organisation and makes key decisions in regard to the allocation of these resources. The Trust is required, as a key target set by the Department of Health, to achieve financial balance at the end of each financial year.

The Trust makes available as much information as possible about the sources, and state, of our finances. In certain circumstances information intended for future publication, audit material, personal information, commercial or confidential information and criminal and regulatory investigative material or law enforcement material will be exempt from publication.

How we purchase supplies and equipment

The Trust follows the guidance established by the NHS Purchasing and Supply Agency (PASA) whose website can be found at http://www.pasa.doh.gov.uk/

The PASA guidance sets out the standard terms and conditions that NHS organisations should follow in regard to purchasing our equipment and supplies. In the NHS we refer to this as procurement.

The Department of Health Commercial Supply Chain Efficiency Programme has a number of strands to address the procurement element of the Gershon Efficiency Review. One strand addresses regional procurement through the creation of collaborative procurement hubs.

The Trust is part of the Trent, Leicester, Northampton and Rutland collaborative procurement hub that came into being on 1 April 2006.

The collaborative procurement hub will address the entire commercial spend of the NHS in Trent, Leicester, Northampton and Rutland. Supplementing procurement activity undertaken by Trusts and PCTs, the hub will undertake sourcing, purchasing and strategic supply chain activities that are optimally managed at a regional level across all categories of expenditure, including specialist areas such as pharmacy, purchased healthcare, estates and capital.

5. Aims, Targets and Achievements

Financial

Every year the Trust prepares a Financial Plan which is presented to the Trust Board for approval before submission to the Strategic Health Authority. The key financial targets which are measured are divided into primary and secondary duties:

Primary Duty

  • To achieve break even on income and expenditure on an annual basis, unless exceptional costs are incurred.


Secondary Duties

  • To achieve a capital cost absorption duty of 3.5% within a range of 0.5% either side

  • Not to breach its External Financing Limit (EFL)

  • Not to breach its Capital Resource Limit (CRL)

There is also a requirement for the Trust to pay 95% of its trade creditors within 30 days. This is a Better Payments Practice Compliance (BPPC).

The Trust is also committed to ensure that best value is obtained wherever possible without compromising quality of patient care.

Performance

The quality of the services we provide and our progress towards meeting the targets set by the Government is monitored through various performance reviews.

The Trust’s performance is reviewed by the Trust Board every month and the reports are available to the public on our website. Details on how to obtain copies are given in Class 2 “ Who we are”

We publish information about waiting times for out patient appointments, day case and inpatient admissions on the National Health Service Website http://www.nhs.uk/

 

How we measure our performance

Governance

Governance is defined by the Audit commission as ‘the systems and processes by which health bodies lead, direct and control their functions, in order to achieve organisational objectives, and by which they relate to their partners and the wider community. The Board ensures that governance issues are managed effectively.

Standards for Better Health

The Department of Health published Standards for Better Health in July 2004. This  comprises a set of 24 essential or ‘core’ standards that all healthcare organisations in England that treat NHS patients should be achieving now, and 13 developmental standards that they should be aiming to achieve in the future.

By assessing compliance against core standards as part of the annual health check, the Healthcare Commission can get an annual overview of how well each organisation is achieving the basic standards of care that the Department of Health requires all healthcare organisations to meet. 

The standards cover seven domains of activity:

  • safety
  • care environment and amenities
  • clinical and cost effectiveness
  • governance
  • patient focus
  • accessible and responsive care
  • public health

The Trust monitors compliance with these standards at all levels within the organisation and reports regularly to the Trust Board and each year to the Healthcare Commission.

Who Monitors the Trust?

The Trust is monitored and scrutinised by a number of external bodies, covering different aspects of its performance, for example professional conduct, services and adverse events. A list of such bodies is attached at Appendix 2.

The HealthCare Commission

The Healthcare Commission promotes improvement in the quality of the NHS and independent healthcare. It has a wide range of responsibilities, all aimed at improving the quality of healthcare. The Commission has a statutory duty to assess the performance of healthcare organisations, award annual performance ratings for the NHS, through the annual health check, and coordinate reviews of healthcare by others.

 

The annual health check is the system for assessing and rating the performance of NHS healthcare organisations in England. It is designed to give the public a more accurate picture of local NHS healthcare organisations, whilst reducing overlap and duplication in the regulation of healthcare providers, as inspections, audits and reviews are focused where they are most valuable.

Non-Clinical services

7. Reports and Independent Inquiries

Class 5 addresses how the Trust is measured by other organisations, which informs how we measure ourselves. Some of these organisations also conduct inspections of our services. We will use this section to inform the public of the results of some of these inspections and investigations.

We will not release the results of any inspection prior to the completion of the inspection process. Similarly, we will not release any information pertaining to an investigation without the agreement of the inspecting organisation. Some information that is of a personal and confidential nature will be excluded from the publication of the inspection reports, as will any other confidential material. Material relating to the health and safety of specific individuals, to law enforcement, criminal or regulatory investigation material or audit issues may also be excluded form publication.

8. Policies and Procedures

The Trust has a large number of policies, which set out the organisation’s rules and provide guidance to staff.

An identified policy author develops new policies. Draft policies are then subject to consultation with the most appropriate people within the organisation, prior to final approval by the relevant committee and / or the Trust Board.  The process is described in the Trust’s policy entitled “Policies and Procedures, Development, Approval, Implementation and Review”.

9. Public Involvement and Consultation

The Trust is committed to a patient centred service and to ensuring that services are responsive to the needs of individuals and communities.  It welcomes feedback from members of the public and service users and seeks to facilitate user involvement tin the development of services at all levels.

Our Statutory Duty

We have a statutory duty to involve service users, carer and the public in the work of the organisation. As stated in class 5, Our Services, we will include this publication scheme in the ways we inform the public of any proposed service changes or variations in accordance with our duties under section 11 of the Health and Social Care Act 2001, which is available directly from the HMSO website www.doh.gov.uk.

Patient and Public Involvement (PPI)

Patient and Public Involvement (PPI) is a term used in the NHS Plan and refers to the national agenda to establish systems that will support service users, carers and community involvement in the development and planning of the NHS. The Commission for Patient and Public Involvement in Health was set up in January 2003.  It is an independent, non-departmental public body, sponsored by the Department of Health. The Commission’s role is to make sure the public is involved in decision-making about health and health services in England.  

Patient and public Involvement (PPI) Forums have been established for every NHS Trust in England to facilitate the continuous engagement of people and communities.  The commission gathers information on opinion from PPI Forums, channelled through its shared information system in order to ensure that the bodies it reports to are acting upon patients’ and the public’s views. The PPI Forums are currently under review.

The PPI Forum

The PPI Forum is an independent body set up by government, with the remit to seek the views of the patient and monitor and review Trust services. There are currently two PPI forums that relate to the Trust, based on the arrangements that applied in regard to the redecessor organisations. Patient Advice and Liaison Service

The PALS service is there during office hours for people, who need advice, have concerns, want to tell us what they think about any aspect of the NHS or don’t know where to turn. PALS helps people to sort out problems on the spot, can give information about services, put people in touch with local or national support agencies and listen to suggestions and comments on how services can be improved.
The PALS gathers the information it receives from service users and feeds this back throughout the Trust so that improvements can be made to benefit the patient experience.

Your Thoughts on Our Service/Patient Feedback Boards

‘Your thoughts on our Service’ / Patient Feedback notice boards are situated around the Trust, which enable anyone to feedback comments about services. These comment sheets are collected and the feedback collated by the ward and department heads. The themes and actions taken around the comments are then displayed on the feedback boards for patient/carers and staff to see.

Training, development and support for PPI activities within the Trust

The PPI team is able to support to teams within the Trust in the development and implementation of their own patient and public involvement initiatives.

Volunteers

The Trust sees the important contribution that volunteers make which enriches and extends the range of service offered to patients. The Trust welcomes volunteers in many of its wards and departments.

10. Regular publications and information for the Public

Information regarding the business processes of the Trust is detailed in Classes 2,3 and 4.

The Nottingham NHS website contains a large amount of information about local NHS services and includes an electronic library designed to address the information needs of patients, public and NHS staff.

The Trust publishes a variety of information on our website for example, how to access our services, information about clinical conditions, self-help groups and many other items of interest.

If you do not have your own internet connection, it is advisable to telephone the relevant department to ask them for the information you require. 11. Complaints

The Trust aims to provide a high quality service at all times. Sometimes things do go wrong and people will wish to complain and have that complaint investigated. It is also useful for the Trust to have people’s views, so they can be taken into account for planning for the future and to improve services for the benefit of the whole community.

The Trust welcomes complaints, compliments, comments and suggestions about the way in which we undertake our work. The Trust’s Complaints Policy and Procedure follow the processes prescribed by law. The Trust is required to provide a written acknowledgment of a complaint within two working days or receipt and, whenever possible, a full response within twenty working days.

The Director of Human Resources is responsible for the human resources strategy, pay and reward policy and personnel matters including medical staff and recruitment, staff training and development.

Policies

Class 8 lists some of the policies the Trust has in place or is developing in regard to Human Resources.


Equity and Equality

The Trust is an equal opportunities employer and aims to eliminate all forms of discrimination. Behaviour regarded as bullying, harassment or victimisation is not tolerated and is dealt with in accordance with the agreed Trust procedures.
Managers ensure that every member of staff is treated fairly and equitably whilst in the employment of the Trust, irrespective of their position or job. Everyone is expected to treat his or her colleagues with respect and dignity at all times.

Communicating with staff

The Trust has effective two-way communication/consultation processes that allow staff to carry out their job effectively. Staff are encouraged to become members of recognised trade unions and staff associations and are consulted on all major issues affecting the Trust.

Managers are responsible for providing staff with information on matters relevant to them and their role.

Staffs are encouraged to express their ideas and opinions and to share information with both their colleagues and managers in an open and honest manner.

We have a weekly, electronic, staff bulletin, which provides brief, up-to-date information and Trust issues and to direct you to other sources of further information, as appropriate. A bi-monthly staff magazine is also published and made widely available.


Staff Involvement

The Trust endeavours to ensure that staff have the opportunity to participate in all decisions which have implications for them, from major organisational change to day to day decisions about how services are delivered. Managers ensure that staff are involved in making decisions within their department/directorate. Staff are expected to participate in the decision-making process when given the opportunity to do so.

Quality of Working Life

The Trust is committed to providing more flexible and supportive working practices, which recognise family and carer responsibilities. We respect everyone’s right to make their own choices about the balance between work and life and recognise that the emphasis will shift as individual circumstances and careers develop. Managers will, wherever possible according to the needs of the service, endeavour to promote a variety of working arrangements that offer staff a choice, flexibility and greater individual control in their working life. Staff are expected to be flexible and adaptable to changing circumstances based on the needs of their colleagues and the necessity to deliver a 24-hour service to patients.

Improving Working Lives (IWL)

The NHS plan makes a commitment to invest in NHS staff. It recognises that a modern NHS must offer staff a better deal in their working lives. Improving the working lives of staff contributes directly to better patient care through improved recruitment and retention – and because patients want to be treated by well motivated, fairly rewarded staff.

The IWL standard has been developed through consultation with staff, unions and professional bodies. It describes the commitment expected from NHS employers to create well-managed flexible working environments that support staff, promote their welfare and respect their need to balance work and home life.

Learning, Education and Development

The Trust provides the necessary training, education and development to enable our staff to carry out their job effectively and to assist them to develop their potential.

Our Managers ensure that staff are provided with regular opportunities to discuss their personal and professional development needs and receive fair access to training, education and development.

Paying our staff

The Pay Services section manages the processes involved in paying our staff. The majority of staff who have pensions are contracted in to the NHS Pensions Agency http://www.nhspa.gov.uk/

Health, Safety and Well Being

The Trust ensures that the health, safety and well-being of all employees are not endangered whilst they are at work and provide services to support healthy lifestyles for all staff. Any form of violence against staff is not tolerated

Managers have the responsibility for health and safety in the workplace and will promote the services which are available to staff whilst in employment.

Staff are expected to take a personal responsibility for the health and safety of both themselves and their colleagues and they are encouraged to maintain a healthy lifestyle. Staff must at all times observe their personal duty of care and relevant professional codes of conduct.

14. Environmental Information

In this class we will publish information about our estate and land holdings and their uses.

This class will also include information required to be in the public domain as a result of Environmental Information Regulations and any Environmental Enforcement actions.

Information about the NHS standards about the Patient’s Environment is available from the NHS Estates website

15. This Publication Scheme

In this class we will publish any changes we make to this Publication Scheme, the criteria on which our information management policies are made and a referral point for all enquires regarding information management generally in the Trust. We will also publish any proposed changes or additions to publications already available.

Cost of Information

For the most part, we will charge you only for hard copies or copying onto media (e.g. CD ROM). Some information is available free of charge, but there may be a charge for other information. The charges will vary according to how information is made available. Charges are as follows:

  • Trust website (www.nuh.nhs.uk) Information is free of charge, although any charges for Internet Service provider and personal printing costs would have to be met by the individual requesting the information

  • Single page printout from the website – available free of charge from the relevant department listed in the scheme. Please note, we will not provide print-outs from other organisation’s websites

  • Multiple page printouts and archived copies of documents that are not available on the website - there may be a charge to cover the cost of retrieval, photocopying and postage. We will let you know the cost and charges that have to be made in advance

  • Leaflets and brochures on services that we offer are free of charge. A list is given in class 11

  • ‘Glossy’ or other bound paper copies, CD Rom, video or other media – a charge will be made for information in these formats. Please email or ring the Executive Business Manager for further information

  • E-mail – information sent by e-mail will usually be free of charge, unless stated otherwise. Where applicable, we will let you know the cost and charges that have to be made in advance.

The charges will be reviewed regularly.
